Viewing your Medical Record Online

Surgeries that choose to offer this service to patients will use Patient' Online Electronic Health Records Access system, to give medical record access from any internet-enabled PC with a web browser. Speak to your own surgery to see if they are going to sign up for the service.

The project - a first in the UK - allows patients to view their records using 'authenticated access', which is strictly controlled by individual passwords and PINs.

Patients can view a summary of their GP's most recent entries, details of consultations, diagnoses, investigations and procedures, results, vaccinations, medication and letters. Practices also benefit, as patients can view letters, results and immunisations without the need to contact a doctor or a receptionist.

Frequently Asked Questions

Why doesn't my sign-in work?

All of the sign-in information is cAsE sEnSiTiVe. So it may be that your CAPS LOCK key is set incorrectly. If you still can't sign in we may be experiencing technical problems so please try again later. If you've done this and still cannot sign-in contact your practice and they will investigate further.

How do I change my password?

If you want to change your password please contact your surgery.

I'm having problems with cookies?

You need to enable cookies to use Patient Access. We keep track of your user session using cookies, so your browser must accept them for the system work work correctly. We don't keep any information about you with these cookies, in fact we send a fresh one every time you return.

Can I sign-in from any computer?

Yes. You can use our online service from any computer. You don't need to re-register. Just be sure to sign-out when you have finished using the service!
